A 39-year-old woman with a history of 5 years of secondary infertility who has a 7-year-old child (from vaginal delivery) underwent laparoscopic laparotomy due to ovarian cysts. On pelvic MRI, a large bilateral pelvic multicellular cyst measuring 135 x 56 mm and a large hydrosalpinx is seen. The left ovary also contains a 47 mm hemorrhagic cyst. In the evaluation of the spouse, the abnormal morphology of the sperm is high (teratospermia), but the number and motility of the sperm are normal.
